Connecting Health Communities 2024 – 2027

Connecting Health Communities, funded by the National Lottery Community Fund, is a 2.5-year initiative supporting cross-sector partnerships to address health inequalities through collaboration between statutory stakeholders, the voluntary sector, and communities facing health disparities.

We worked with nine areas in Phase 1 (2021 – 2023) and four additional areas are receiving 24 month intensive support during Phase 2 (2023 – 2025). We have now selected four areas for Phase 3, each with a unique health inequality focus and partnership goals aimed at impactful, community-driven outcomes.
